庐山市人民医院成功开展首例CRRT技术,为危重患者点燃生命之光!

</p><p>近日,我院成功开展了第一例床旁连续性肾脏替代(CRRT)治疗。本次病例为一名尿毒症并长期行血透的患者,因肺部感染血氧下降入住我院ICU,入院诊断:重症肺炎 呼吸衰竭 慢性肾功能衰竭(CKD5期),给予经鼻高流量吸氧治疗后,患者指脉氧维持在95%左右,但是患者在血透过程中出现血压下降导致透析效果不佳,且出现精神行为异常。重症医学科主任周燕带领团队联合肾内科主任周继生及副主任钱小明针对患者的病情进行多学科讨论及全面评估后考虑患者符合CRRT治疗指征,告知家属并取得同意后,决定对患者行CRRT治疗。在治疗期间患者血压平稳,神志逐渐清楚,经过长达22个小时的持续肾脏替代治疗后患者成功下机,下机后患者生命体征平稳,神志清楚,对答切题。复查血肌酐值由666.5umol/L下降至167.2umol/L,血气分析指标基本正常。</p><p </p><p>何为CRRT技术</p><p> </p><p> </p><p>连续性肾脏替代治疗(continuousrenalreplacementtherapy,CRRT)是指一组体外血液净化的治疗技术,是所有连续、缓慢清除水分和溶质治疗方式的总称。</p><p>传统CRRT应持续治疗24h以上,但临床上可根据患者的治疗需求灵活调整治疗时间。CRRT治疗目的不仅仅局限于替代功能受损的肾脏,近来更扩展到常见危重疾病的急救,成为各种危重病救治中最重要的支持治疗措施之一。</p><p /></p><p> </p><p>图为连续血液净化设备(CRRT)</p><p> </p><p>CRRT技术的适应证</p><p> </p><p> </p><p>1.肾脏疾病</p><p>重症急性肾损伤:伴血流动力学不稳定和需要持续清除过多水或毒性物质,如 AKI 合并严重电解质紊乱、酸碱代谢失衡、心力衰竭、肺水肿、脑水肿、急性呼吸窘迫综合征、外科术后、严重感染等。</p><p>慢性肾脏病并发症:合并急性肺水肿、尿毒症脑病、心力衰竭、血流动力学不稳定等。</p><p>2.非肾脏疾病</p><p>包括多器官功能障碍综合征、脓毒血症或感染性休克、急性呼吸窘迫综合征、挤压综合征、乳酸酸中毒、急性重症胰腺炎、心肺体外循环手术、慢性心力衰竭、肝性脑病、药物或毒物中毒、严重容量负荷、严重的电解质和酸碱代谢紊乱、肿瘤溶解综合征、热射病等。</p><p> </p><p> </p><p>血气分析</p><p> </p><p> </p><p>血气分析(BG)是应用血气分析仪,通过测定人体血液的H+浓度和溶解在血液中的气体(主要指CO2、O2),来了解人体呼吸功能与酸碱平衡状态的一种手段,它能直接反映肺换气功能及其酸碱平衡状态。采用的标本常为动脉血。</p><p>适用于:低氧血症和呼吸衰竭的诊断;呼吸困难的鉴别诊断;昏迷的鉴别诊断;手术适应证的选择;呼吸机的应用、调节、撤机;呼吸治疗的观察;酸碱失衡的诊断等。</p><p
